Should you have changed your address please inform us accordingly without delay. Only when your actual address is in our files can it be ensured that important letters sent to you by your compensation authorities will also reach you.

Please note that this original form signed by yourself must be sent to your compensation authorities. It is not adequate to send a letter by e-mail or fax!

Note: Should your compensation benefits be paid by cheque, you must state your full postal address without fail. Just giving a c/o address is not permitted.
